ROY, Madison Leslie - It is with immense sadness that Phil and Tara (née Kuchma) announce the passing of their little angel Madison, aged 8 days, passed peacefully in her parents arms at McMaster Children's Hospital NICU on May 31, 2012. Taken from us far too early, she was loved by all who met her and will never be forgotten. Baby Madie is loved and will be deeply missed by her parents Phil and Tara; grandparents Jan, Nicole, Cléo, Libby, Ron and Christine; great-grandma Sherry; aunties Mélanie, Jarilyn (Roger), Jaclyn and Michele. Madison will always remain her parents baby girl and they will hold her again one day in Heaven.
A private family service will be held at the Beckett & Glaves Family Funeral Centre on Monday, June 4, 2012. Donations in Madison's memory in lieu of flowers can be made to the McMaster Children's Hospital NICU.
Thank you to all the incredible doctors, nurses and staff at McMaster who helped us share 8 special days with our beautiful girl. A tree will be planted in memory of Madie in the Beckett-Glaves Memorial Forest.
